Scheduling at Nourishing Life Collective
- Complete this form (HIPPA compliant) to help our Front Desk staff transition your care to the new clinic
- Release of Information and NLC New Patient Form
- Once we receive your form, our Front Desk staff will complete your patient portal access for NLC through AthenaHealth. 
- My Current Schedule: M 1:30pm-6:30pm, Tu 9:30a-5:30p, Th 8:30a-1:30p

- The clinic is open 5 days a week, 9a-5p.


*OHP/Trillium Patients:
- I will start seeing all OHP/CareOregon patients over at Nourishing Life Collective.
- I am working to get contracted with Trillium, OSHU Medicaid and Kiser Medicaid. 
- For patients with CareOregon, we will continue care as usual, but I will need to create a NEW Charm Account for you. You will be receiving notifications for this simple process.
- For now, continue to schedule online via our current Charm Portal!


Accepting Medicare:
- We are accepting Medicare Advantage at Nourishing Life Collective!
- Medicare insurance plan: Regence MedAdvantage (Part C) insurance

Transition Dates:
- Through August 21st, all appointments will take place at the Opening to Life Building (3942 SE Hawthorne Blvd)
- If you already have an appointment already scheduled AFTER August 21st, you will be contacted to set your appointment up with the Nourishing Life Collective and your appointment will remain the same time, but at the new clinic!

Medical Records 
- If you wish to transition your care to another provider and would like your medical records:
- Log in to the CharmHealth Patient Portal
- Go to your dashboard
- Select the medical records or documents section
- Choose your preferred date range
- Click the download or print option to save your files.
